Post-acute analytics solution firm Real Time Medical Systems has partnered with Kentucky-based Signature HealthCARE to implement the firm’s interventional analytics throughout all of Signature’s skilled nursing facilities.

The company offers services in eight states across the continuum of care, including skilled nursing, rehabilitation, assisted living, memory care, in-home care, cognitive care and telemedicine.
Real Time’s data-driven clinical and financial solutions will enable Signature’s facilities to achieve greater efficiency, deliver high-quality care and improve care coordination with their healthcare partners, said Joan Neuscheler, CEO of Baltimore-based Real Time. Receiving a 95% KLAS-Rating among its nursing facilities, health system, ACO, and payer customers, Real Time’s interventional analytics solution has been shown to reduce readmissions, detect early signs of infection, and improve care transitions across a patient’s post-acute care journey.
