As the granddaughter of several survivors of the Holocaust, Rose Trevino, LCSW, CDP, has seen firsthand how the most vulnerable populations can fall through the cracks, receiving minimal help to navigate resources. As a result, she has devoted her life’s work to connecting older adults with limited resources to the services they need to live meaningful lives with dignity.
Trevino, director of operations for Chicago-based Help at Home, has a Bachelor of Arts degree in psychology and a Master of Social Work degree, both from Aurora University. In 2012, Gott was selected as a Kott Gerontology Scholar, awarded to advanced graduate students who have a focus on aging.
She previously worked at MPAC Healthcare, AMITA Health and Oak Street Health, as well as served as an adjunct faculty member at Aurora University.
In 2021, as director of operations for Help at Home, Trevino helped launch a transformative care coordination business segment connecting home care to healthcare. The HealthyMatch program has shown success relating caregiver observations to a clinical team and a network of providers and community resources to create a more holistic model of care for older adults. One of the program’s biggest benefits is that it has helped improve quality, care and cost outcomes by addressing unmet medical, behavioral and social needs, especially among those with complex chronic care conditions.
After training the frontline workers in Help at Home’s many branches on how to integrate the program into their work, results suggest it already is having an effect. The program has helped narrow the gap for controlled blood pressure by 30% in one market, among other statistics. The program also has identified thousands of intervention points that have prevented avoidable adverse incidents and allowed older adults to remain in their homes.
