PointClickCare Technologies is acquiring Collective Medical, a platform that supports real-time, cross-continuum care coordination.
The deal, close to being finalized at press time, will bring new services to PointClickCare’s 21,000 clients — and expands the cloud-based software provider’s footprint outside of post-acute care.
The combined technologies are expected to provide more insights at any stage of a patient’s healthcare journey, enabling better decision-making and improved clinical outcomes while potentially lowering costs.

The companies first partnered in 2019 to streamline the integration of Collective Medical’s care transitions solutions with PointClickCare’s software platform and influence decisions at the point of care. The Collective Medical platform supports patients with complex conditions, particularly those requiring multiple interventions and transitions across disparate care settings. PointClickCare expects that added expertise will enable improved care for the most complex, high-cost/high-needs patients.
In the U.S., 97% of hospitals discharge patients to skilled nursing facilities using PointClickCare. Collective Medical’s platform connects more than 1,300 hospitals, thousands of ambulatory practices and long-term post-acute care providers, accountable care organizations, and every national health plan in the country across a 39-state network. Collective Medical is based near Salt Lake City, UT.
The acquisition was in the $500 million to $1 billion range, PointClickCare said.
